Overview

Support shaped around the person

Medication routines can become difficult when prescriptions, timings or refills begin to feel overwhelming. We agree a clear support plan based on current clinician instructions, the person’s abilities and the professional scope required.

Before care begins, the team confirms needs, scope, location, availability and the practical expectations of everyone involved.

Can a caregiver give or administer medication?

A caregiver may provide reminders and limited practical assistance where appropriate. Medication administration is assigned only to an appropriately qualified professional and must follow the agreed care plan and current clinician instructions.

Can your team change a dose or medicine?

No. We do not prescribe, stop or change medication. Questions about a medicine or dose are referred to the prescribing clinician or pharmacist.
